Nurse Case Manager

SNI Companies is currently hiring Nurse Case Managers  in Cleveland Ohio.This role involves guiding patients through their healthcare journeys, providing education and support, and identifying opportunities to reduce unnecessary healthcare costs. This is a direct hire opportunity.

? Candidate Requirements

• Active RN license required (no LPNs)
• 2+ years of case management experience strongly preferred
• Strong documentation, computer, and phone communication skills
• Experience educating patients on chronic conditions and providing support throughout treatment plans
• Familiarity with cost containment strategies and understanding of insurance processes
• Ability to engage patients who may be reluctant to participate in case management

? Work Environment

• In-office presence required for training period
• After training: flexibility for hybrid or fully remote schedule
• Open to candidates outside the immediate Cleveland area
